The Medication History Technician will play a crucial role in ensuring the accuracy of patient medication lists. This position requires strong communication and organizational skills to effectively gather and convey medication information to healthcare providers.

Key Responsibilities
  • Prioritize workflow to focus on high-priority patient types
  • Obtain accurate home medication histories through chart review, patient interviews, and phone calls to pharmacies and physician offices
  • Process patient medication history and enter information accurately into the computer system
  • Update electronic home medication databases and compare lists to admission orders
  • Collaborate with nursing, pharmacists, and physicians to resolve discrepancies
  • Collect and enter complete allergy information, including reaction types
  • Utilize medication information resources to identify medications as needed
  • Accurately collect data for reporting to hospital quality committees
Requirements
  • Virginia Board of Pharmacy technician licensure or Licensed Practical Nurse in the Commonwealth of Virginia
  • Current PTCB certification preferred
  • Minimum of two years of Pharmacy Technician or LPN experience
  • Strong mathematics and interpersonal skills
  • Ability to work with computers and manage multiple tasks
