About the Job

This is a per diem opportunity for a LPN to work in a home health setting. You will be responsible for providing nursing care to patients in their homes, including administering medications, wound care, and other medical treatments. You will also be responsible for documenting patient information and communicating with healthcare teams to ensure seamless care.

Responsibilities
  • Provide nursing care to patients in their homes, including administering medications, wound care, and other medical treatments.

  • Collaborate with other healthcare professionals to develop and implement patient care plans.

Requirements

  • Current licensure as a LPN in the state of practice.
  • Current BLS/CPR certification from the American Heart Association (AHA) or the American Red Cross (ARC).
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
