Key Responsibilities- Provide direct patient care, including assessment, diagnosis, planning, and implementation of nursing interventions.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in multidisciplinary treatment planning and revise plans of care based on assessment.
- Document patient information accurately and thoroughly, including progress notes, medication administration records, and other relevant data.
- Graduate of an accredited School of Nursing.
- Current Illinois license.
- B.S.N. preferred or equivalent years of experience.
- BLS certification required.
- Demonstrates clinical competence.
- Assumes responsibility for own continued personal and professional growth.
- Ability to operate standard office equipment (i.e. copier, facsimile machine, 10-key, etc.).
- Personal computer and printer and specific computer software (MS Office applications and Meditech).
- Requires grasping and reaching with hands/arms.
- Ability to lift up to, but not limited to 25 pounds when assisting clients, turning clients or assisting client with activities.
- Requires bending, stooping, and walking.
- Requires good body mechanics to prevent injury.
