Key Responsibilities- Assess, plan, implement, and evaluate the care needs of a designated group of patients or provide nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
- Provide professional nursing care to patients, requiring a nursing license.
- Oversee the design, implementation, or delivery of processes, programs, and policies using specialized knowledge and skills typically acquired through advanced education.
- Manage projects/processes, work independently with limited supervision, coach and review the work of lower-level professionals, and resolve difficult and sometimes complex problems.
- Develop and maintain collaborative relationships between all members of the healthcare team, inclusive of administrative, clinical, research, and inpatient partners, to produce high-performing, patient-focused teamwork.
- Act as a resource for all staff in regards to education, planning, relocating, and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population.
- Mentor and actively participate in overseeing all clinical support staffing in the ambulatory clinical setting.
- Identify high-risk patients and work with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
- Promote shared decision-making with patients in regards to healthcare; educate the patient and/or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions.
- Utilize a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
- Collaborate and actively engage with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.
- Bachelor's degree in Nursing (BSN)
- Associates degree considered with review of Nurse Executive Council
