We are seeking a skilled Registered Nurse to join our team as an MDS Coordinator at Avamere Riverpark of Eugene. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.
Responsibilities- Review patient care plans to ensure they are aligned with individual needs and goals.
- Collaborate with healthcare professionals to ensure seamless care delivery.
- Schedule and facilitate care plan conferences to promote effective communication.
- Monitor and guide the completion of PPS and OBRA assessments to ensure accurate data.
- Manage the Utilization Review (UR) process to optimize resource allocation.
- Oversee medication administration records (MARs), treatment flow sheets, and physician orders to ensure accuracy and compliance.
- Participate in facility surveys to maintain regulatory compliance.
- Current, unencumbered, active license to practice as a registered nurse in this state.
- Nursing degree from an accredited college or university.
- MDS Certification preferred.
- Experience with Electronic Medical Records and computer documentation systems.
- Knowledge of nursing and medical practices, laws, regulations, and guidelines.
