Position Overview:
An individual trained to assist, under professional guidance, patients with nutritional and environmental support, personal hygiene, feeding, and dressing, as well as select health-related tasks. The Personal Care Aide reports to the supervising Registered Nurse overseeing the case for patient care and to the Staffing Coordinator/Team Leader for administrative responsibilities.

Qualifications:

  • Ability to read and write in accordance with job requirements.
  • Successful completion of an accredited basic personal care aide training program consistent with New York State regulations, or
  • One full year of experience providing personal care services through a home care agency within the last three years.

In cases where the personal care aide is tasked with health-related duties, appropriate training must be completed prior to patient assignment, supported by written documentation.

Must provide evidence of freedom from health issues that could harm the patient, along with a documented health status assessment completed before starting patient contact and annually thereafter, or as needed. A pre-employment physical examination by a licensed physician or physician assistant is required, along with annual health assessments.

Documentation of the following is necessary:

  • Immunization against rubella, evidenced by serologic proof or documentation of vaccination.
  • Negative tuberculosis (TB) test results within the last 12 months; those without proof must undergo testing.
  • Immunization against Rubeola for individuals born after January 1, 1957, or medical exemption documentation.
  • Immunity to Hepatitis B or appropriate medical exemption.
  • All employees must be current with Covid-19 vaccinations, with medical exemptions accepted.

Additional requirements include:

  • Drug screening prior to employment and annually thereafter.
  • A statement confirming no addiction to depressants, stimulants, narcotics, alcohol, or other drugs.
  • Background checks prior to employment.
  • A valid driver's license is preferred.

Responsibilities:
- Assist patients in maintaining a clean and hygienic living environment.
- Help ensure a safe and healthful atmosphere.
- Perform basic record-keeping as directed by the supervising nurse.
- Aid in patient transfers and positioning.
- Support patients with personal hygiene tasks, including bathing, dressing, grooming, and toileting.
- Follow the established care plan diligently.
- Engage in light housekeeping duties, such as changing bed linens, dusting, and tidying up.
- Attend in-service training and staff development programs for at least 6 hours annually.

Evaluation:
Performance assessments will be conducted by the Staffing Coordinator/Team Leader, incorporating feedback from supervisory and administrative personnel.
