The Registered Nurse (RN) Case Manager is a vital member of the healthcare team at TriStar Centennial Medical Center. This role is responsible for coordinating patient care, managing length of stay, and ensuring appropriate resource utilization. The RN Case Manager will work closely with the multidisciplinary team to develop a safe and effective discharge plan, promoting quality outcomes and patient throughput.
Key Responsibilities- Provides case management services for inpatient and observation patients, identifying those at risk for adverse outcomes during transition from one level of care to another.
- Performs comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as resources available to them.
- Reassesses patients' clinical conditions as needed, considering readmission status or risk and developing strategies to mitigate.
- Coordinates the plan of care and drives the discharge plan by collaborating with the healthcare team and patient's physician to facilitate a successful care transition.
- Partners with Social Services to ensure post-acute medical needs and level of care are appropriate.
- Assumes responsibility for timely referral to Social Services when risk factors for psychosocial determinants of health are identified.
- Involves patients and their families in identifying and clarifying needs and expectations to develop mutual and realistic goals.
- Evaluates progression of care using evidence-based tools and approved criteria (InterQual) throughout the episode of care, escalating issues through the established chain of command.
- Makes appropriate referrals to third-party payer and disease and case management programs for recurring patients and those with chronic disease states.
- Facilitates patient throughput with an ongoing focus on effective care transition, quality, and efficiency.
- Associate Degree in Nursing or Nursing Diploma
- Bachelor's Degree in Nursing
- 2+ years experience in case management or 3+ years experience in clinical nursing
- InterQual experience
