Key Responsibilities:
- Individualized Education: Develop and implement individualized education programs for residents, focusing on their specific condition and treatment options.
- Treatment Plan Development: Collaborate with licensed mental health professionals to create and implement effective treatment plans, ensuring residents receive necessary support and guidance.
- Assessment and Evaluation: Conduct thorough assessments to identify resident needs, developing and implementing crisis intervention plans as needed.
- Documentation and Record-Keeping: Maintain accurate records of resident progress, including treatment plans, assessment results, and other relevant information.
Requirements:
- Education: High school diploma or equivalent required; related experience and/or training in pre-release coordination, group/individual educational programming, and case management preferred.
- Experience: 1 year of work experience in pre-release planning and coordination, with demonstrable experience working with individuals with behavioral/mental health disorders/illnesses.
- Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and proficiency in case management methods and techniques.
