- Complete care plans and other documents as assigned, maintaining accurate and up-to-date records.
- Comply with HIPAA and corporate guidelines, maintaining confidentiality and upholding patient trust.
- Supervise, direct, and provide resident care as assigned, ensuring a safe and supportive environment.
- Establish personal rapport with patients, families, and other individuals, representing The Grand Rehabilitation & Nursing with professionalism and respect.
- Provide support and information to patients, families, and healthcare teams, utilizing effective communication and interpersonal skills.
- Maintain documentation of patient care services, auditing records to ensure accuracy and quality.
- Active and unrestricted NYS Registered Nurse License.
- Excellent clinical assessment skills and ability to make informed decisions.
- Computer proficiency in MS Office (Excel, Word, Outlook) and ability to adapt to new technologies.
- Long-term care experience preferred but not required.
- Excellent communication and interpersonal skills, with a focus on patient-centered care.
