Key Responsibilities- Collaborate with the care team to develop and implement individualized care plans, ensuring optimal health outcomes and efficient resource utilization.
- Conduct regular patient care rounds and multidisciplinary discharge planning conferences to assess continuing care needs and improve communication among the patient healthcare team.
- Interview patients and family members to assess their needs and preferences, and coordinate with nursing to provide patient education and support.
- Coordinate timely discharge plans, arranging follow-up care and linking patients with community resources as needed.
- Communicate effectively with physicians, nurses, and other healthcare professionals to ensure seamless care transitions.
- Monitor and analyze indicators such as length of stay, delays in services, and quality outcomes to identify opportunities for improvement.
- Stay up-to-date on industry best practices and maintain a network of professionals to exchange ideas and benchmark best practices.
This position requires a strong foundation in nursing practice, effective communication skills, and a patient-centered approach. The ideal candidate will be a registered nurse with experience in case management or a related field, and a strong understanding of the principles of growth and development over the life span.
