Overview

A Healthcare Access Coordinator provides essential support to patients and healthcare professionals within a designated area, aiming to enhance access to care, facilitate transitions, and coordinate patient care resources.

This position collaborates with a multidisciplinary team that manages patients with complex medical or social needs.

The Healthcare Access Coordinator actively engages with patients and their families both in clinical settings and via phone, employing critical thinking and effective communication to identify obstacles that may hinder patients from achieving optimal health outcomes.

This role is pivotal in ensuring timely access to medical services and connecting patients and families with professional and community resources, empowering them to take an active role in their healthcare management.

The Healthcare Access Coordinator also offers additional support and education to patients recently discharged from medical facilities.

Meetings with patients may occur in various locations, including their residences or community sites.

Responsibilities include coordinating patient appointments, managing medical prior authorizations, and conducting follow-up outreach for patients facing challenges in attending appointments.

The Healthcare Access Coordinator collaborates closely with care managers to enhance clinical efficiency.

Key Responsibilities

(60%)* Employs motivational interviewing techniques to uncover social risk factors that may impede patients from achieving their healthcare objectives. Conducts necessary clinical screenings to identify patient risk factors. Connects patients with community and medical resources to address social determinants of health and barriers to care. Coordinates medical appointments, assists patients in overcoming medication adherence challenges, and actively works to improve appointment attendance, while also coordinating follow-up with non-clinical services such as transportation and home health.
(20%)* Completes all necessary workflows and documentation for assigned patients in a professional manner. Communicates urgent patient care needs to the appropriate team members promptly.
(15%)* Educates patients on the effective use of healthcare resources. Contributes to the development and management of patient care plans by collaborating with care managers, providers, and other team members based on patient interactions.
(5%)* Participates in quality improvement initiatives and specific clinical tasks aimed at enhancing patient access.

Additional Duties

The Healthcare Access Coordinator will follow up with patients recently discharged from medical facilities or emergency departments to identify barriers affecting their healthcare goals. They will assist patients requiring authorization prior to scheduling appointments and facilitate access to specialty care by coordinating appointments with relevant departments.

Additionally, they will educate patients on community resources to help eliminate barriers to healthcare access.

They will reach out to patients who have missed appointments to encourage future compliance and work with them to develop strategies for overcoming challenges related to attending visits.

Exceptional customer service and communication skills are essential.

Provides education on healthcare and community resources.

Demonstrates clinical competencies annually.


Certifications and Licenses

BLS - Basic Life Support Required within three months of hire.
MA CERT - Medical Assistant Required.
All Medical Assistants must possess one of the following active certifications: Registered Medical Assistant (RMA), Certified Medical Assistant (CMA), or equivalent credential from an accredited organization.

Education

High School Diploma or GED required.

Experience, Training, and Skills
Assists in maintaining a safe and clean environment for patients and colleagues. May require travel to various locations for patient visits and community resources.

Maintains confidentiality and ensures patient rights in compliance with HIPAA regulations.

May assist in completing tasks related to the patient's healthcare plan, such as filling out forms and communicating with community organizations.

Adheres to organizational policies and electronic medical record workflows to ensure documentation is accessible for quality initiatives.

Provides age-appropriate care according to departmental standards.

Exhibits empathy and resourcefulness towards all patients, regardless of their social or medical complexities.

Demonstrates multitasking abilities and effective communication skills.

Works collaboratively with healthcare team members and navigates the healthcare system efficiently.

Must thrive in a dynamic healthcare environment as a valued member of the multidisciplinary team.

Ability to follow written and verbal instructions, review reports, and prioritize tasks effectively.

Practices within the scope of medical certification and performs other related duties as assigned.


Physical and Mental Requirements

Ability to manage stressful situations, including public contact and sensitive circumstances. Must be able to wear personal protective equipment as required.

Must be able to move freely within the facility and perform job functions as outlined.

Overall vision and hearing capabilities are necessary, with or without assistive devices.

Frequent sitting, standing, and walking for extended periods may be required, along with postural changes.

Some exposure to potentially infectious materials; adherence to safety protocols is mandatory.

Ability to handle multiple tasks, maintain regular attendance, and stay alert throughout the workday.

Physical demand level: Light. Must be able to occasionally lift or carry up to 20 lbs.
