Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Services for the UnderServed (SUS). As an LPN, you will play a vital role in coordinating medical and psychiatric care for residents, ensuring proper documentation, follow-up, and regulatory compliance. Your expertise will be essential in hands-on training of residential staff to maintain ongoing and current compliance.

Responsibilities:

Coordinate all medical and psychiatric care for residents, ensuring proper documentation and follow-up
Assist with hands-on training of residential staff to maintain ongoing and current compliance
Identify and organize appointments for the week in conjunction with the RN and Manager
Transport and accompany residents on scheduled medical appointments and complete necessary follow-up upon return
Perform delegated nursing tasks as directed by the RN and under the supervision of the RN
Oversee coordination of all medication administration, including verifying prescriptions, ordering medications, regulatory documentation, and staff administration of medications in conjunction with the RN and under supervision of the RN
Maintain medication inventory, including prescription medications, over-the-counter medications, and required first aid supplies
Ensure maintenance of all medically related documentation (MAR, MD Consults, etc.)
Update the RN daily on resident medical status
Complete all documentation prior to the end of the shift and report all issues/concerns to the RN and Residence Manager
Complete the documentation of Occurrence Reports as required
Review and make daily entries into the House Log Book on all pertinent resident/residence-related issues
Monitor resident progress and notify applicable others of concerns, changes in behavior, or progress
Implement all directives issued by the RN and Manager as needed
Handle telephone calls and inquiries, provide necessary follow-up, and keep family and management notified when necessary
Know and apply emergency procedures in the event of a crisis, including fire safety
Act as liaison between Residence, Day Program, Physicians, and families on all resident medical-related issues
Provide individual residents with instruction and/or assistance with daily living skills and rehabilitation or social skills in accordance with the Individual Service Plan
Provide ongoing training to residents and staff in areas of need identified in the Individual Service Plan in conjunction with the RN and Manager
Provide hands-on client-specific training to staff members as directed by the RN
Attend annual and semi-annual reviews, providing medical input in the absence of RN involvement
Visit residents in the hospital as needed and coordinate Discharge Plans in conjunction with the RN
Complete necessary residential habilitation notes, progress notes, and other required documentation
Comply with OSHA regulations and universal precautions, AMAP Training, annual recertification training, and annual PPD clearance as per agency policy
Attend various meetings/in-services, participate in staff meetings in conjunction with mandatory annual in-services as directed by the Residence Manager or RN
Supervise residents' intake of medication as prescribed in accordance with OMRDD-approved Medication Administration Procedure
Maintain a friendly and professional working atmosphere
Maintain a professional appearance
Ensure residents are always treated with dignity

Qualifications:

SPECIAL SKILLS :
