Position Overview

SUMMARY: The Nutrition Specialist is tasked with providing dietary guidance for patients identified with nutritional vulnerabilities, particularly focusing on individuals with obesity and diabetes. This role also encompasses responsibilities as a Diabetes Care and Education Consultant, managing a diverse patient panel within a collaborative, team-oriented healthcare environment.

KEY RESPONSIBILITIES

1. Individual Nutrition Counseling

  • Conducts comprehensive assessments and follow-up consultations utilizing the Nutrition documentation in the Electronic Medical Record (EMR).
  • Develops personalized nutrition strategies that establish achievable goals for patients, supplemented with educational materials and resources.
  • Ensures patients receive a written summary of their goals to facilitate ongoing progress.
  • Exhibits expertise in specialized areas of nutrition.
  • Utilizes sound clinical judgment in treatment recommendations in collaboration with primary healthcare providers.
  • Evaluates the psychological needs of patients and makes referrals to mental health services as necessary.
  • Identifies social needs of patients and coordinates referrals to social support services.
  • Addresses spiritual needs and provides appropriate spiritual care, including sharing faith-based support when suitable.
  • Practices cost-effective healthcare by considering the necessity of tests and treatments.

2. Group Educational Sessions

  • Facilitates group educational workshops on targeted topics for patients and community organizations, utilizing existing or newly developed lesson plans and handouts.
  • Effectively communicates and educates participants regarding nutrition.
  • Clearly articulates the role of a nutrition specialist to group attendees.

3. Diabetes Education and Support

  • Collaborates with primary care providers to deliver additional education and support for diabetic patients during dedicated sessions.
  • Offers training on home glucose monitoring techniques using finger-stick and continuous glucose monitoring devices.
  • Provides instruction on insulin administration and GLP-1 injections.
  • Coordinates monthly group medical consultations.
  • Monitors patients adjusting insulin dosages and provides guidance based on established protocols.
  • Engages in population management and data analysis.
  • Reports on productivity and key diabetes metrics to supervisors and relevant stakeholders.
  • Works with the Medical department to incorporate Community Health Workers into diabetes care initiatives.

4. Community Health Initiatives

  • Advocates for nutrition and healthy lifestyle choices at community events, health fairs, and educational classes, preparing necessary materials for these engagements.

5. Maternal and Infant Nutrition Education

  • Collaborates with obstetric teams to provide essential breastfeeding education and nutritional counseling for expectant and postpartum mothers.

6. Professional Development and Communication

  • Maintains open communication with colleagues regarding patient care, programs, and operational procedures.
  • Seeks guidance and feedback from supervising physicians as needed.
  • Responds promptly to urgent patient needs.
  • Communicates effectively with external healthcare professionals.
  • Proposes enhancements and contributes to the implementation of practice improvements.
  • Engages in continuous learning through formal and informal educational opportunities.
  • Performs additional duties as assigned by supervisors.

QUALIFICATIONS

  • Bachelor's degree in Food and Nutrition or Dietetics from an accredited institution, including completion of the required supervised practice or internship as mandated by the Academy of Nutrition and Dietetics (AND).
  • Registered with the Commission on Dietetic Registration (CDR) at the time of employment or within six months of hire, and must obtain state licensure as a Licensed Dietitian-Nutritionist (LDN) upon registration.
  • Bilingual in Spanish and English.
  • Ability to maintain composure in high-pressure situations.
  • Strong verbal communication skills; proactive in ensuring clear and direct communication with team members.
  • Proficient in computer applications and Microsoft Office Suite.
  • Demonstrated ability to manage multiple tasks and prioritize effectively.
  • Proven track record of handling confidential information with integrity and trustworthiness.
  • Ability to integrate personal faith with professional practice.

WORK ENVIRONMENT

  • Requires proficiency in using computers, reading fine print, and navigating electronic health records.
  • Must be capable of lifting or moving up to 40 pounds and performing occasional physical activities such as stretching, reaching, stooping, or kneeling.
