RN Long Term Care Specialist

Amergis is seeking a skilled Registered Nurse to join our team in a Long Term Care setting. As a RN, you will be responsible for providing high-quality patient care and collaborating with the healthcare team to ensure the best possible outcomes.
Key Responsibilities- Assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility
- Perform focused interviews to identify specific patient needs
- Collect, analyze, and interpret data and information from healthcare team members and document actual and/or potential nursing diagnoses
- Develop and implement individualized patient plans of care
- Provide a safe, comfortable, and therapeutic patient environment
- Initiate and evaluate patient and/or family education
- Use clinical judgment in evaluation activities to meet patient care needs
- Current Registered Nurse License for the state in which the nurse practices
- Complies with all relevant professional standards of practice
- Participation and completion of Amergis' Competency program when applicable
- Current CPR if applicable
- TB Questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state, and local requirements
- Successful completion of new hire training as applicable to job site
- Understand patient confidentiality and HIPAA requirements
- Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills
