Job Opportunity:

A Clinical Care Specialist is needed for a 91-day assignment in Louisville, Kentucky. As a Medical Transition Coordinator, you will provide specialized care for patients transitioning from critical care to general medical-surgical units.

Responsibilities:
  • Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status.
  • Administer medications, perform procedures, and collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Educate patients and families on post-intensive care recovery, medication management, and self-care techniques.

Requirements:
  • Current RN license
  • BLS (Other Certifications as required by facility)
  • Two years of recent Step Down experience
  • Strong communication and adaptability skills
