About the Role

We are seeking a skilled Vocational Rehabilitation Counselor to join our interdisciplinary team at Collage Home and Community Rehabilitation Partners. As a key member of our team, you will play a vital role in providing exceptional neurological rehabilitation services to our patients.

Key Responsibilities
  • Assess patients and families to identify coping abilities, adjustment to disability, behavior, and relationships.
  • Develop and implement individualized treatment plans to address psychosocial needs.
  • Provide counseling, support, education, and resources to patients and care partners.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Conduct regular progress assessments and adjust treatment plans as needed.
Requirements
  • Master's degree or higher in professional counseling or a related field from an accredited college/university.
  • Completion of specific coursework and a 300-hour supervised practicum with at least 100 hours of direct client contact.
  • Appropriate licensure in the state of residence.
  • Experience working with adults with acquired brain injury, behavioral intervention, cognitive rehabilitation, staff training, and computer literacy.
  • Ability to assist patients confined to wheelchairs and who utilize assistive devices.
