Job Title: LPN Nurse

Job Summary: We are seeking a skilled LPN Nurse to join our team at a long term care, memory care, short-term rehabilitation and senior independent living facility. As an LPN Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities:

  • Administer medications and treatments in accordance with established standards and protocols
  • Provide safe and efficient personal care to residents, including bathing, dressing, and grooming
  • Use recommended safety devices and follow infection control standards to prevent the spread of illness
  • Respond to resident calls and requests for assistance in a timely and professional manner
  • Document medications and treatments administered, as well as care provided, in a clear and concise manner
  • Participate in resident care planning and update care plans as needed
  • Assess residents and document findings, reporting to the physician as necessary
  • Transcribe physician orders and communicate resident changes to resident sponsors
  • Destroy and document discontinued medications
  • Maintain accurate controlled substance numbers during shift
  • Complete required documentation of nursing notes, transfer forms, and other records
  • Assist with the operation of the unit, including the monthly change over of records
  • Complete and distribute nurse aide assignments
  • Provide report to on-coming shift
  • Schedule and assist with physician, optometrist, podiatrist, and dentist appointments
  • Administer and record employee health testing results

Requirements:

  • Indiana Professional License or License from an NLC participating state required at time of submission
  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records
  • Hands On - American Heart Association CPR and BLS Certification required
  • Dementia 6 hour certification is required prior to start
  • LTC experience required

Preferred Qualifications:

  • EMR, Matrix Care experience preferred
  • Experience with Memory Care
  • Experience with short-term rehab

Physical Demands:

  • Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state and local standards
  • Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc.
  • Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor
