Job Summary:

You will collaborate with patients, families, physicians, and other healthcare professionals to identify patient needs and coordinate services to meet those needs.

Key Responsibilities:

  • Provide physical therapy services to patients with physical impairments resulting from injury, disease, disability, or other causes.
  • Collaborate with patients, families, physicians, and other healthcare professionals to identify patient needs and coordinate services.
  • Document patient progress and treatment plans.
  • Travel to patients' homes within the PHH geographic area.

Requirements:

  • Two years of experience in acute care, rehab, or homecare as a Physical Therapy Assistant.
  • Licensed as a Physical Therapist Assistant in Massachusetts.
  • Proficient computer skills and ability to learn other software applications.
  • Excellent customer service skills and ability to work with diverse populations.
  • Strong verbal, written, and electronic communication skills.
