Job Summary

We are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Elara Caring, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals and improve their quality of life.

Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs and goals.
  • Provide one-on-one physical therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
  • Collaborate with other healthcare professionals, including nurses, therapists, and social workers, to ensure comprehensive care and support for patients.
  • Communicate effectively with patients and their families, providing education and support to help them understand their treatment plans and progress.
  • Document patient progress and outcomes, using standardized tools and measures to track and evaluate the effectiveness of care.
Requirements
  • Current Physical Therapy license in good standing.
  • CPR certification with American Heart Association or American Red Cross.
  • 1 year of experience in a healthcare facility, preferably in a home health care setting.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for long periods.
  • Positive attitude and strong communication skills.
  • Dedication to providing high-quality patient care and support.
