Key Responsibilities:
- Deliver exceptional patient care and assessments, adhering to established practice standards and clinic policies.
- Conduct thorough patient evaluations, summarize clinical findings, and formulate precise individualized care plans.
- Administer treatments in alignment with physician directives and the established care plan.
- Assess the effectiveness of treatment strategies and modify care plans as necessary.
- Engage in effective communication with patients, families, and healthcare team members to optimize care outcomes.
- Collaborate with referral sources to ensure comprehensive patient care and progress updates.
- Identify educational needs of patients and their support systems to enhance care delivery.
- Exhibit proficiency in clinical techniques and procedures relevant to the patient populations served.
- Prepare and implement appropriate discharge plans for patients.
- Evaluate safety risks and implement necessary precautions.
- Adhere to established security and safety protocols.
- Utilize leadership skills to delegate tasks and educate team members effectively.
- Contribute to quality improvement initiatives within the clinic.
- Optimize patient scheduling and time management to meet efficiency standards.
- Ensure timely and compliant documentation using AgileRPM.
- May require travel to various local clinics as needed.
Qualifications:
- Master's or Doctorate degree in Physical Therapy.
- Valid Physical Therapist license in accordance with state regulations.
Physical Requirements:
- Must be able to perform essential job functions, with reasonable accommodations for individuals with disabilities.
- Frequent standing, walking, sitting, reaching, climbing, and other physical activities are required.
- Ability to lift and/or move up to 25 pounds regularly and occasionally lift/move more than 100 pounds.
