Job Summary

We are seeking licensed practical nurses (LPNs) to join our team of dedicated healthcare professionals. As an LPN with BAYADA Home Health Care, you will have the opportunity to work in a variety of settings, including client homes, hospitals, and other healthcare facilities. Our LPNs play a critical role in delivering high-quality care to our clients, working closely with our registered nurses and other healthcare professionals to ensure seamless care transitions and optimal outcomes.

Responsibilities
  • Provide high-quality, patient-centered care to clients in their homes, hospitals, and other healthcare settings
  • Work collaboratively with registered nurses and other healthcare professionals to develop and implement individualized care plans
  • Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing these needs
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians
  • Document client progress, including vital signs, medication administration, and other relevant information
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care transitions and optimal outcomes
Qualifications
  • Current LPN license in good standing
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to lift, push, and pull up to 25 pounds
  • Ability to work in a fast-paced environment and adapt to changing situations
