Job Title: RN Case Manager

Job Summary:

Loyola Medicine seeks a dedicated and compassionate RN Case Manager to join our team. As a key member of our multidisciplinary team, you will work closely with physicians, staff, and payers to ensure seamless patient care coordination and effective communication.

Responsibilities:

  • Conduct thorough patient assessments to identify complex clinical discharge needs and develop individualized plans of care with the multidisciplinary team.
  • Facilitate daily multidisciplinary rounds to identify and address care coordination barriers, ensuring efficient and cost-effective care.
  • Collaborate with nursing staff to develop evidence-based, outcomes-oriented care plans and provide education to patients and families on available care options.
  • Communicate effectively with patients, families, and healthcare teams to ensure patient-centered care and facilitate smooth transitions throughout the care continuum.
  • Maintain accurate and timely documentation of patient care and progress, ensuring compliance with regulatory requirements and organizational policies.

Requirements:

  • Bachelor's Degree in Nursing or equivalent experience
  • 3-5 years of previous job-related experience, with a focus on case management or clinical care coordination
  • Recent clinical experience in medicine, surgery, critical care, telemetry, or ED required; recent case management experience preferred
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Ability to maintain confidentiality and handle sensitive information with discretion
