Job Summary:

We are looking for a dedicated and compassionate Nurse Practitioner to provide medical house calls to our patients in their homes. As a member of our team, you will work collaboratively with our scribe/medical assistant, nurse, social worker, and other resources to ensure seamless care coordination and address the unique needs of our patients.

Key Responsibilities:
  • Conduct medical house calls for homebound patients according to defined pathways
  • Optimize and monitor clinical status, identify and address gaps in care, reconciling medications and addressing adherence challenges
  • Collaborate with home health, specialists, social workers, hospitals, SNFs, and specialists
  • Ensure clinical excellence, compliance with regulatory guidelines, and promotion of the Oak Street Health care model
Requirements:
  • Genuine passion for primary care and reaching under-resourced communities
  • Strong clinical skills focused on older adults with complex disease states
  • Experience in home-based care (as RN or APN)
  • Excellent communication, collaboration, and problem-solving skills
  • Desire to be a part of a fast-paced, innovative team
  • Active, non-probationary state medical license, including US work authorization, if applicable
  • 1+ years APN experience, preferably with a Medicare population
  • Access to reliable transportation
  • Flexibility to travel around the local market
  • Bilingual (English and Spanish) preferred, but not required
  • Working knowledge of Word, Excel, and EMRs
  • US work authorization
