Key Responsibilities:
- Manage a defined patient caseload with a focus on quality care.
- Provide direct nursing services and support to individuals in their homes.
- Collaborate with healthcare teams to ensure optimal patient outcomes.
Qualifications:
- Current nursing license in the state of New Hampshire.
- Minimum of 2 years of experience in skilled nursing care preferred.
- At least 1 year of nursing supervisory experience preferred.
- Recent experience in wound care is essential.
- Ability to lift up to 50 pounds without restrictions.
- Current CPR certification is required.
- Reliable transportation is necessary for community visits.
