Per-Diem LPN, Hospice Care Specialist

Job SummaryUnder the supervision of a Registered Nurse (RN), the Per-Diem LPN provides nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families. This role requires a high level of clinical skills, including medication administration, symptom management, and patient relationships.
Responsibilities- Provides nursing tasks to patients and families, including Continuous Care, dressing changes, and catheter changes, as identified in the plan of care developed by the Interdisciplinary Group (IDG).
- Demonstrates clinical skills regarding application of Company policies and procedures, such as medication administration, symptom management, and patient/family relationships.
- Observes and reports signs of adverse reactions and responses to treatments and reports to RN.
- Reports findings to physicians, APRNs, and physician's assistants following collaboration with RN.
- Performs venipuncture for blood specimens and other duties as assigned.
- A high school diploma or G.E.D. or an equivalent combination of education and experience.
- Current LPN licensure in the state of Florida or current licensure from another state plus a temporary Florida permit.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- Minimum of one (1) year nursing experience in an in-patient, hospice, or home health setting.
- Mobile Driver - Valid driver's license and automobile insurance per Company policy.
- Reliable transportation to meet visit schedule.
- Ability to communicate (written and verbal) in English.
- Flexible schedule to meet the needs of patients.
- Ability to use equipment with visual and auditory mechanisms.
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
Satisfactorily complete competency requirements for this position.
