Hospice Nurse Opportunity in Erie County

Flexible Scheduling

We offer flexible scheduling, including Monday - Friday daylight shifts with rotations for on-call and weekends. You will support various locations in Erie and surrounding areas, providing personalized patient care in a home setting.

As a member of our team, you will collaborate with our team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to provide comprehensive patient care. Benefits and Incentives

Responsibilities

As a hospice nurse, your responsibilities will include establishing patients' plans of care in collaboration with physicians, patients, and utilization management (UM) professionals. You will coordinate case management activities across a multi-disciplinary health care team to ensure appropriate, quality patient care. You will also initiate, deliver, and evaluate patient- and family-centered health teaching, striving to help patients achieve optimal health outcomes, delivering compassionate care with an emphasis on the patient's experience.

Requirements

We are seeking a registered nurse with zero to one year of experience. Completion of UPMC student nurse internship, nurse residency, or student clinical transitions course at UPMC is preferred. BSN is also preferred. You must be able to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. You must also be able to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Critical thinking, analytical and problem-solving abilities are required as related to various aspects of patient care. Good clinical judgment and critical thinking skills are necessary to exercise and to lead others in the application of the nursing process. You must also have mobility and visual manual dexterity, as well as physical stamina for frequent walking, standing, lifting, and positioning of patients.

Licensure and Certifications

You must have current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located OR TPP RN license. You must also have CPR certification based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Additionally, you must have an automotive insurance, driver's license, Act 33, Act 34, and OAPSA.
