We are seeking a Licensed Practical Nurse (LPN) to join our team at Samaritan Summit Village, a 168-bed skilled nursing facility located in Northern NY. As an LPN, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.
**Responsibilities:**
* Provide direct care to residents, including administering medications, performing treatments, and documenting patient information
* Work collaboratively with the nursing team to develop and implement individualized care plans
* Participate in ongoing education and training to stay up-to-date on best practices and industry developments
* Maintain accurate and timely documentation of patient information
**Requirements:**
* Current NYS LPN license
* BLS certification (American Red Cross or American Heart Association)
* Ability to work a variety of shifts, including days, evenings, and weekends
* Strong communication and interpersonal skills
