Discharge Planner Job Description

The GEO Group is seeking a highly skilled Discharge Planner to join our team in Campbell, CA. As a Discharge Planner, you will be responsible for developing and implementing discharge plans to support participants' successful transition back into the community.

Key Responsibilities

  • Develop and implement discharge plans that address participants' identified needs and goals.
  • Collaborate with Community-Based Providers (CBPs) to ensure seamless delivery of services and coordinate participant placements.
  • Monitor participant progress relative to their discharge plans through regular meetings and data tracking.
  • Maintain accurate records of participant interactions and updates in the ARMs electronic database.
  • Collaborate with Parole Agents to ensure timely and effective communication regarding participant progress.
  • Participate in ongoing training and professional development opportunities to enhance your skills and knowledge.

Requirements

  • Bachelor's degree from an accredited university or equivalent experience working with criminal justice populations.
  • Valid driver's license required.
  • Ability to recognize the importance of family, social networks, and community systems in treatment and recovery.
  • Knowledge of program documentation requirements, ethical standards, and professional conduct.
  • Ability to facilitate participant engagement in treatment and recovery processes.
  • Possession of case management skills necessary for discharge planning, multi-disciplinary teamwork, and assessment of treatment planning efficacy.

Preferred Skills and Experience:

  • Possession of Alcohol and Drug Counselor Certification recognized by the Department of Health Care Services (DHCS) preferred.
  • Understanding of cognitive behavioral therapy and criminogenic risk factors.
  • Ability to conduct assessments and evaluations for CBP placement.
  • Ability to provide culturally sensitive services and establish rapport with participants.

Salary Range:$55,000 - $65,000 per annum.
