- We are seeking a highly skilled clinical practitioner to support our busy Internal Medicine team.
- The ideal candidate will provide urgent, same-day or next-day access to patients.
- Responsibilities will include providing interim co-management of chronic illnesses, titrating medications, and conducting routine follow-up care.
- Additionally, the successful candidate will conduct CMS Annual Wellness Visits as needed and ensure continuity of care during transitions.
- We offer a dedicated 21-month onboarding program to support the practitioner's success.

Qualifications
- AANP or ANCC certification in family or adult geriatric nursing, or NCCPA certification as a physician associate.
- Experience working in a busy primary care clinic is essential.
- The ideal candidate will be comfortable caring for complex and co-morbid patients.
- Excellent customer service skills and the ability to work independently and as a collaborative team player are required.
