Job Summary:
We are looking for a skilled and dedicated professional to fill the role of Clinical Liaison. The successful candidate will be responsible for providing exceptional patient care, collaborating with physicians and the rehabilitation team, and developing and implementing nursing care plans. About the Role:
- Provide quality care for patients and serve as a role model for other nursing staff.
- Collaborate with physicians and the rehabilitation team to coordinate and deliver rehabilitative services to all subacute and residential patients.
- Interface nursing care standards of practice with the interdisciplinary team model.
- Assist in the development, implementation, and evaluation of nursing care plans and interdisciplinary care plans via the MDS 2 Plus format.
- Serve as a resource for nurses at all levels and for other health care providers for advanced clinical practice in a specific area of expertise.
- Practice standard precautions at all times and follow specific precautions when applicable.
- Evaluate the clinical outcomes of nursing practices, resource utilization, and environmental conditions.
- Perform admissions to the subacute unit.
- Demonstrate effective decision-making for efficient matching of demands and resources to resolve clinical and professional issues.
- Consult with all nursing staff on issues related to clinical problem-solving and nursing care planning.
- Consult with members of the substance team on issues related to medical and rehabilitative care.
- Consult with patients, families, and potential patients on issues related to subacute care. Visit referral sources as appropriate.
- Contribute to nursing knowledge through scholarly publications and professional presentations on clinical topics and issues covered in nursing.
- Assess learning needs and design, implement, and evaluate comprehensive teaching programs for specific patient populations, healthcare providers, and community groups to improve patient outcomes.
- Facilitate the acquisition and application of clinical knowledge, theoretical knowledge, and decision-making skills by nurses and other healthcare providers.
Requirements:
- Registered Professional Nurse licensed in the state of MA.
- Graduated from an accredited master's degree in nursing.
- Minimum five years of experience in acute care nursing.
- Minimum two years of experience in staff development/nursing education.
- Awareness of current trends in nursing, standards of care, and legal issues.
Estimated Salary: $80,000 - $110,000 per year, depending on experience.
