Job Summary:

We are seeking a skilled RN Case Manager to join our Home Health team. As a key member of our team, you will be responsible for planning, organizing, and directing home care services. Your expertise in nursing, with a focus on community health education, will enable you to build strong relationships with patients and families, ensuring they receive the best possible care in the comfort of their own homes.

Responsibilities:
  • Conduct initial assessments of patients and families to determine home care needs.
  • Develop and implement individualized care plans, making necessary revisions as patient status and needs change.
  • Administer medications and treatments as prescribed by physicians.
  • Supervise and evaluate home health aide care provided every two weeks.
  • Identify discharge planning needs as part of the care plan development.
  • Maintain confidentiality and adhere to Federal HIPAA guidelines.
  • Demonstrate proficiency in OASIS completion.
  • Complete documentation through EPIC in accordance with agency timeliness policies.
Requirements:
  • Active, valid Massachusetts RN license.
  • Active American Health Association BLS certification.
  • Associates Degree or BSN (preferred) from an accredited school of nursing.
  • At least 1 year of direct care nursing experience, preferably in public health or home care nursing; OASIS and/or EPIC experience is preferred.
