Job Summary:

MLee Healthcare Staffing and Recruiting, Inc is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services. As a Physical Therapist - Home Health Specialist, you will be responsible for conducting thorough evaluations of patients in their home settings, developing and implementing individualized therapy programs, and delivering physical therapy directly within patients' homes.

Key Responsibilities:

1. Patient Assessments:
Conduct thorough evaluations of patients in their home settings to identify their physical capabilities, limitations, and rehabilitation needs.

2. Work Schedule:
80 hours biweekly, Monday through Friday.

3. Tailored Treatment Plans:
Develop and implement individualized therapy programs based on patient assessments, medical histories, and physician referrals.

4. In-Home Therapy Services:
Deliver physical therapy directly within patients' homes, which may include exercise programs, manual therapy, and techniques for improving mobility.

5. Education for Patients and Families:

Teach patients and their families about exercise techniques, therapeutic approaches, and lifestyle modifications to support recovery and enhance functional abilities.


6. Progress Tracking and Plan Adjustments:
Regularly monitor and document patient progress, adjusting treatment plans as needed to achieve optimal outcomes.

7. Collaborative Care:
Partner with physicians, healthcare providers, and caregivers to ensure coordinated and effective care for patients.

8. Emergency Management:
Address urgent situations or emergencies that may occur during home visits, providing appropriate care and interventions as necessary.

Requirements:

1. Physical Therapist License:
Current physical therapist license in the state of practice.

2. Education:
Doctor of Physical Therapy degree from an accredited program.

3. Experience:
At least 1 year of experience in home health care or a related field.

4. Skills:
Excellent communication and interpersonal skills, ability to work independently, and strong problem-solving skills.
