LPN Certified

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Silverton of Cascadia. As an LPN, you will play a vital role in providing high-quality care to our residents, working under the direction of an RN and collaborating with the healthcare team to ensure the best possible outcomes.

The LPN (Certified) will provide person-centered nursing care and treatment to elderly, acutely ill, or injured residents. This includes initiating the SBAR process, collaborating with the RN for assessment and recommendations, and participating in resident evaluation and care planning activities.

• Provides direct care and/or assists with care as directed and consistent with their scope of practice and competency.
• Reviews, transcribes, communicates, and implements physician orders, obtaining RN signature as required by state regulations or company policy.
• Documents care performed and observations of resident status in the clinical record promptly.
• Participates in medication management to promote optimal safety and effectiveness for residents.
• Assists with emergencies, administers cardiopulmonary resuscitation, and completes clinical admission paperwork for new admissions.
• Performs other clinical care services within scope of practice for additional certifications.

• Graduate of accredited LPN School of Nursing.
• Current valid LPN license in the state employed.
• Valid CPR certification.
• Current certification in one of the following clinical specific areas: Long-term care, Gerontology, Wound care, IV therapy, Ventilator, Pulmonary & Tracheostomy, or Hospice & Palliative care.

• Six months experience in a long-term care environment.
