As a key member of our multidisciplinary team, you will have the opportunity to take a hands-on approach to patient care, addressing their needs and understanding the impact of social determinants on health outcomes.
Key Responsibilities- Provide high-quality, patient-centered care to 12-15 patients per day
- Collaborate with a team of experienced professionals, including Advanced Practitioners and a Care Team
- Participate in daily huddles to discuss patient care and medical expertise
- Manage a Medicare Advantage panel of patients with a focus on coordinated care and value-based relationships
- Stay up-to-date with Medicare guidelines and HEDIS quality indicators
- Doctor of Medicine or Osteopathic Medicine degree
- Valid, unrestricted license in the state of desired employment
- Current, unrestricted DEA license
- Minimum of two to five years of directly applicable experience
- Board Certified/Board Eligible in Family, Internal, or Geriatric Medicine
- Guaranteed base salary with bi-annual quality incentive
