We are seeking a highly skilled and compassionate Social Services Therapist to join our Clinical Services team at Foundations for Living. As a key member of our team, you will play a vital role in providing exceptional care and support to our patients and their families.
Responsibilities- Provide individual and group therapy sessions to patients with a range of mental health conditions, including anxiety, depression, and trauma.
- Develop and implement personalized treatment plans that address the unique needs and goals of each patient.
- Collaborate with multidisciplinary teams, including psychiatrists, nurses, and other therapists, to ensure comprehensive care and support.
- Conduct assessments and evaluations to identify patient needs and develop effective treatment strategies.
- Provide education and support to patients and their families on mental health topics, including coping skills, stress management, and relapse prevention.
- Masters degree in Social Work or a related field.
- Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
- At least two years of experience in a mental health setting, preferably in a clinical or therapeutic role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Proficiency in Microsoft Office and electronic medical records systems.
