We are currently seeking a MDS/Clinical Reimbursement Coordinator to join our dedicated team at CareOne.
Key Responsibilities:
- Oversee the Resident Assessment Process by facilitating the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAA's).
- Engage in the development of personalized resident Care Plans, identifying individual needs and expected outcomes in compliance with Federal and State regulations.
- Conduct MDS/RAI Care Plan Functions.
- Complete nursing-assigned MDS items, CAA's, and Care Plans as directed by the facility.
- Participate actively in resident Care Plan Meetings.
- Current RN licensure in the state of practice with experience in PPS and OBRA assessments.
- Prior experience in a hospital, long-term care facility, or other healthcare environment.
- Strong knowledge of general, rehabilitative, and restorative nursing practices, as well as the laws and regulations governing long-term care.
- Proficient in computer skills, including Windows applications; experience with Accu Med is preferred.
- Demonstrated experience in completing MDS and Care Plans is essential.
- 1-3 years of MDS experience or RN with long-term care experience who can be trained in assessment roles.
- Able to work independently and manage time effectively.
- Flexible schedule to accommodate month-end close processes.
- Detail-oriented with the ability to complete assessments accurately and promptly.
