Job Description
The RN/LPN MDS Coordinator is responsible for conducting thorough assessments of residents' medical conditions, developing individualized care plans, and collaborating with our healthcare team to identify areas of improvement.

Responsibilities

  • Conduct comprehensive assessments of residents' medical conditions
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Ensure accurate and timely completion of Minimum Data Set (MDS) assessments
  • Monitor and report any changes in residents' medical status
  • Collaborate with the healthcare team to identify areas of improvement and implement changes

Requirements

  • Associate's degree in Nursing (ADN) or Bachelor's degree in Nursing (BSN)
  • Current RN or LPN/LVN licensure in the state of employment
  • RAC-CT certification preferred
  • At least 1 year of experience in a nursing home or long-term care setting
  • Excellent communication and interpersonal skills
