Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Patient Centered Care Specialist. In this role, you will be responsible for providing comprehensive patient-centered care to patients with complex medical needs.

The ideal candidate will have strong assessment and critical thinking skills, excellent communication abilities, and a passion for delivering exceptional patient care.

Key Responsibilities:

  • Provide comprehensive patient-centered care to patients with complex medical needs.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Collaborate with physicians, nurses, and other staff to ensure quality patient outcomes.
  • Communicate regularly with patients and families to ensure they understand their care plan and any changes.
Requirements

To be successful in this role, you will need:

  • A degree in Nursing (BSN or higher) from an accredited program.
  • At least one year of experience in a healthcare setting, preferably in a patient care coordination role.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • CPR/BLS certification.
