We are seeking a highly skilled Clinical Care Specialist to join our team at LHC HomeCare of South AL. As a Licensed Practical Nurse, you will be responsible for providing quality patient care and performing technical skilled care in the patient's home.

The ideal candidate will have current LPN licensure in the state of practice, a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Additionally, they will possess CPR certification and have experience working in home health settings.

Key Responsibilities:

  • Provide services within the scope of practice as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Instruct patients' families on patient care and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
