Clinical Navigator Job Description

PAM Health is seeking a skilled Clinical Navigator to join our team. As a Clinical Navigator, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments.

Key Responsibilities:
  • Develop and maintain strong relationships with referral sources, including physicians, discharge planners, and case managers.
  • Conduct on-site pre-admission assessments and provide clinical information to obtain pre-certification from payers.
  • Coordinate and complete phone/FAX assessments, and document pre-admission assessments legibly, accurately, and completely in the approved format.
  • Review medical records, identify aspects related to pre-certification financial coverage and admission criteria, and serve as a navigator to the Admissions office.
  • Communicate with facility Admissions department and facility CEO on a daily basis to optimize census management and case finding strategies.
  • Follow up on all pending as well as medical denials in person to re-evaluate potential for admission.
  • Conduct follow up with referral facilities to ensure satisfaction and service levels were met.
Requirements:
  • Degree from an accredited college or university in a health field, preferably nursing.
  • Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy.
  • Minimum of two years' experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
