About the Role

Maximus is seeking a skilled Field Registered Nurse Assessor to provide high-quality home assessment services to vulnerable populations in the Bronx. As a key member of our team, you will conduct assessments to determine eligibility for community-based long-term services, utilizing the UAS-NY assessment tool.

Key Responsibilities
  • Travel to consumer homes, facilities, or conduct telehealth assessments as needed.
  • Perform in-home, in-facility, or telehealth assessments to evaluate individual health status, strengths, care needs, and preferences.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Regularly review medical documentation and report any concerns or issues that may arise during the evaluation process.

Requirements
  • Current Registered Nurse (RN) license valid in the state of practice.
