We are seeking a skilled Nurse Practitioner to join our team at Trinity Health PACE. As a key member of our healthcare team, you will provide primary medical management to our participants, working collaboratively with our esteemed PACE Organization (PO) Physicians.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to ensure participants receive high-quality care.
- Collaborate with PO physicians to implement care plans and provide acute illness treatment.
- Participate in discharge planning and provide support to participants and their families.
- Conduct nursing home or home visits as needed to provide comprehensive care.
- Assist in formulating clinical policies and procedures to ensure best practices.
- Participate in provider on-call rotation to ensure 24/7 coverage.
- Master's degree in Nursing.
- 5-7 years of experience in long-term care, preferably in a PACE setting.
- Active license to practice in North Carolina.
- CDS Certification, DEA Registration, and Board Certification required.
- Current BLS certification.
- Ability to travel within the Center's geographic region.
