VNS Health is seeking a skilled and experienced Per Diem Field Supervisor RN to join our team in Manhattan, New York.
Job Summary:The Per Diem Field Supervisor RN will be responsible for monitoring and assessing the delivery of home health field services, supervising the activities of Home Health Aides, and providing case management support, clinical intervention, and development of care plans as needed.
Key Responsibilities:- Perform initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, in-home coordination, and crisis management.
- Collaborate with primary care physicians and clinical teams to perform full patient assessments, gathering medical history and medication information.
- Supervise initial home patient assessments to ensure Home Health Aides are following VNS guidelines and regulations.
- Identify continued training needs of Home Health Aides and document information electronically.
- Deliver start of care packets and provide overall reviews with patients/caregivers on initial visits.
- Participate in quality assurance and education programs as requested by management.
- License and current registration to practice as a Registered Professional Nurse in the State of New York required.
- CPR/BLS Certification required.
- Associate's Degree in Nursing from an approved program required.
- Bachelor's Degree in Nursing from an approved program preferred.
- Minimum of two years in a clinical background as a Registered Nurse required.
- Home care experience in a licensed home care service agency or CHHA settings preferred.
