Responsibilities
  • Develop and implement strategies to improve medication management practices and reduce errors.
  • Collaborate with other departments to enhance patient outcomes and quality of care.
  • Design and deliver educational programs for pharmacy staff, students, and residents to promote professional growth and development.
  • Participate in departmental quality improvement initiatives to identify areas for improvement and develop solutions.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.

Requirements
We are seeking a highly skilled and experienced pharmacist who has graduated from an accredited College of Pharmacy with a B.S. or PharmD degree. A minimum of two years of relevant experience specific to practice area is required, along with completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience. Current Connecticut Pharmacist License or eligibility for licensure in the state of Connecticut is necessary.
