We are seeking a skilled and experienced Home Health Care Physical Therapist to join our team at Interim HealthCare. In this role, you will provide care for clients in their homes, working independently to assess and treat patients with various physical therapy needs.

Job Summary

This is a part-time position that requires the ability to work under pressure and prioritize tasks effectively. You will be responsible for conducting comprehensive assessments, developing treatment plans, and implementing interventions to improve patient outcomes.

The ideal candidate will have current licensure as a Physical Therapist (PT) in the state of Colorado, as well as one year of experience in PT. Home health care experience is preferred but not required. A background check and drug test are mandatory, and a Basic Life Support (BLS) certification is required or willing to obtain upon hire.

We offer a competitive salary, ranging from $75-$150 per visit, plus mileage and travel time compensation. Our benefits package includes medical, dental, and vision coverage, as well as supplemental benefits, 401(k) matching, and paid time off.

Responsibilities
  • Conduct initial and ongoing comprehensive assessments of patients
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Provide interventions and therapeutic treatments to patients
  • Communicate effectively with other healthcare team members to ensure seamless coordination of care
  • Participate in quality assessment and performance improvement initiatives
