Job Summary

ProHealth Home Health & Hospice is seeking a highly skilled Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their place of residence.

Key Responsibilities
  • Develop and implement individualized plans of care in collaboration with patients, families, and healthcare providers.
  • Conduct comprehensive assessments and evaluations to identify patient needs and develop effective care strategies.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and families.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
  • Participate in quality improvement initiatives and contribute to the development of best practices in home health care.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • Minimum of 2 years of experience in home health care or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
