LPN - Clinic II (Meds) - Healthcare Professional

As a Licensed Practical Nurse (LPN) in our Clinic II (Meds) team, you will provide direct and indirect nursing care in an outpatient setting. Under the direction of a Registered Nurse and medical orders from a Practitioner, you will review and prepare medical records, initiate follow-up, and identify patient needs. You will also implement basic nursing procedures, administer treatments, and provide laboratory and x-ray reports to Practitioners for review.
Key Responsibilities:
- Review and prepare medical records for assigned patients, ensuring accuracy and completeness.
- Implement basic nursing procedures, including administering treatments and medications as prescribed by Practitioners.
- Provide laboratory and x-ray reports to Practitioners for review, and notify patients of normal results as directed.
- Collaborate with RNs and Practitioners to revise care plans as needed, and demonstrate competency in basic nursing skills and equipment use.
- Assign basic nursing tasks to unlicensed personnel as appropriate, and provide clear directions and follow-up to ensure tasks are completed per standards.
- Manage patient vaccine data, including updating the Hawaii Immunization Registry System and Electronic Health Records.
- Identify patient immunization requirements, fill out immunization paperwork, and facilitate vaccine inventory control.
- Train new and existing LPNs on emergency vaccine storage procedures, and serve as a liaison with the state to coordinate VFC inventory.
- Assist in ensuring compliance with infection control policies and protocols, and provide educational materials to patients.
- Participate in orientation of new personnel, and demonstrate knowledge, skills, and abilities necessary to provide care and/or service appropriate to age groups served.
Requirements:
- Licensed Practical Nurse (Hawaii) required at hire.
- Basic Life Support from American Heart Association.
- Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
Preferred Qualifications:
- Previous nursing experience.
- Previous clinic experience.
- Demonstrated knowledge of and skill in word processing and spreadsheet PC applications.
