We are seeking a highly motivated and dedicated Rehabilitation Specialist to join our team at Houston Circle of Hope Services, Inc.

About the Job

The successful candidate will be responsible for providing evidence-based training and support services to adults with complex needs. This will involve working closely with clients, families, and other stakeholders to develop and implement personalized service plans that promote independence and self-sufficiency.

As a key member of our rehabilitation team, you will be expected to provide high-quality support services, including skill-building training, crisis intervention, and documentation of client progress. You will also be required to maintain effective relationships with clients, families, and other professionals to ensure seamless delivery of services.

Key Responsibilities
  • Skill-Building Training: Provide one-on-one skills sessions in the home or community setting, focusing on goals and objectives using evidence-based practices and curriculums.
  • Crisis Intervention: Offer emergency support services through face-to-face contact, phone calls, or an on-call system, documenting all crisis hours spent assessing, planning, linking, and stabilizing clients.
  • Documentation: Maintain accurate and up-to-date electronic records, writing progress notes in accordance with Texas Administrative Code requirements.
  • Communication: Utilize active listening skills to establish rapport with clients and their families, providing emotional support and encouragement, and engaging family members in the service process.
  • Record-Keeping: Enter data accurately and timely, submitting necessary documentation for tracking output/outcome measures as needed.
Initial Requirements
  • Educational Background: A Bachelor's Degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • Experience: At least one year of paid experience in a related field.
  • Certification: ANSA/CANSA Certification is preferred but not required.
What We Offer
