Home Health Patient Care Manager Assistant LPN Job Description

We are seeking a skilled and compassionate Home Health Patient Care Manager Assistant LPN to join our team at Brookdale Home Health St. Louis. As a key member of our home health care team, you will play a vital role in ensuring the continuity of high-quality care to our patients.

Key Responsibilities:
  • Assist the Patient Care Manager in coordinating a team of staff to provide quality care to patients in accordance with physician's orders and State/Medicare guidelines.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
  • Assist in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.
Requirements:
  • Current CPR certification.
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
