Job Description

We are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Care Professional. As a RN, you will be responsible for providing high-quality nursing care to clients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:
  • Provide skilled nursing care to clients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop and implement individualized care plans to meet those needs.
  • Collaborate with our interdisciplinary team, including therapists, social workers, and other healthcare professionals, to ensure comprehensive care.
  • Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
  • Current valid nursing license and graduation from a qualified nursing program.
  • CPR in good standing.
